What You Will Learn

The diagnosis is made before the dressing is chosen, and most of it is made in the history. This module teaches the assessment that every wound plan depends on: a wound-relevant history that captures duration, prior treatment, comorbidity, medications and the barriers that decide whether a plan can be carried out; a systematic vascular, neurologic, musculoskeletal and skin examination, including Doppler waveforms, the ankle-brachial index and its calcification trap, and the 10 gram monofilament; reproducible wound measurement and description of the wound bed, exudate, edges and periwound skin; bedside recognition of infection in the patients where fever and white cell count fail; and the selective use of laboratory tests, cultures, imaging and biopsy, including the osteomyelitis workup. It ends where assessment should, with a defended etiology and a judgment on whether the wound is healing or has stalled.
